This Mother’s Day, we’re celebrating the love, strength, and beauty of moms with something truly special — a Smile Makeover Giveaway at Northwest Center for Dentistry!
We’re inviting our community to nominate a deserving mother who has always put others first and could benefit from a renewed sense of confidence. Whether it’s a friend, family member, or someone you admire in the community, this is your chance to give her the gift of a brighter, healthier smile.
How It Works
Step 1: Enter to Win or Nominate a Mom
Fill out the short nomination form linked below. Tell us why you or a mother you know could benefit from having a smile makeover and how it could have a positive impact.
Step 2: Meet the Finalists
Our team will review submissions and select finalists who embody compassion, resilience, and selflessness. We are also selecting the patient based on need, and not only personal attributes.
Step 3: Transform a Life
The winner will receive a personalized smile makeover by Dr. Clark and our expert team — designed to restore health, function, and beauty.

What’s Included:
The winner will receive a comprehensive treatment plan tailored to their needs. This may include:
- Professional teeth whitening
- Porcelain veneers
- Dental bonding
- Crowns or other cosmetic restorations
Treatment details will be based on clinical recommendations and patient eligibility.
Contest Phases & Timeline
Phase 1: Nominations Open
📍 May 5–16
We’ll accept nominations for deserving moms via the official contest post comments.
Phase 2: Announce Top 12
📍 May 22
Our team will review all nominations and select the Top 12 based on the following criteria:
• Impact of their story
• Need for cosmetic dental care
• Alignment with the spirit of the contest
Phase 2: Announce Top 6
📍 June 1st
Announce the top 6 who are selected to come in for their complimentary exams based on the following criteria:
• Phone interview
• Willingness to take photos and videos
• Dental health assessment
Phase 3: Announce Top 3 and Start Voting
📍 June 15th
From the Top 6, we will narrow down to 3 finalists. These 3 finalists will be selected based on:
• The findings during the complimentary dental exam and interview at our office
• Consent to photos and videos to use on our social media platforms
Phase 4: Announce winner
📍June 22nd
We’ll narrow the finalists to the Top 3, and announce the Grand Prize Winner on our social media and website!
Phase 5: Grand Reveal

Eligibility Requirements
- Age: Nominees must be 18 years or older.
- Residency: Must reside in Flathead Valley, Montana.
- Health: Nominees must be in good general health and free from conditions that would prevent safe cosmetic dental treatment.
- Dental Screening: Finalists must complete a complimentary dental exam and x-rays to confirm eligibility. Conditions such as advanced decay, gum disease, or need for extensive procedures may disqualify a nominee.
- Consent: Finalists must agree to the use of their name, image, and testimonial in promotional materials.
- You must need to have permission of the mother that you are nominating.
- Prize Cap: Prize covers cosmetic dental treatment up to $16,000 in value. Additional procedures (if needed) are the winner’s responsibility.
Legal Compliance
- This contest is void where prohibited and subject to all applicable federal, state, and local laws. Northwest Center for Dentistry reserves the right to disqualify any entry that does not meet requirements or shows signs of fraudulent activity.
